Start: March

13-week assignment with the option to extend

Shift Details: 7:00 - 3:30 (no later than 5pm some days), No nights, no weekends, no call

Schedule: PRN

Work Details:
· Amount of coverage needed per month: Variable
· Case Mix: Opthamology, general, ENT, joints, podiatry, GI, Ortho, plastics, some regional and ANES take care of regional blocks. NO heads, spines, or hearts.
· CRNAs WILL have an opportunity to work solo

Practice Setting:
• Critical Access Facility

Requirements:
· License required: IL license in hand

Credentialing Timeframe: 2-3 weeks once full file has been received - Please set expectation that they be responsive and send items over ASAP as timeline does not start until full file is received.
Full travel and lodging covered
